Asbestos Abatement Contractor Registration

To complete the Asbestos Abatement Contractor Registration, you will need electronic copies of the following:

A list of all federal, state or local violations you have received regarding construction/asbestos containing materials within the previous 2 years
A copy of your current IDPH Contractor's license
A signed and notarized copy of required Child Support Affidavit(s). One per owner is required.
Credit card or electronic check information for payment of fees
$200 Application Fee
